“SIM-card replacement”

To get the new SIM-card you need to:
1. Contact the service center life:).
2. Provide a phone number of lost SIM-card.
3. Provide documents for identifying the subscriber:
∙ for individual subscribers - passport of contract’s owner;
∙ for corporate subscribers - an application for SIM-card issue in case of loss, certified by the signature of the head and chief accountant, as well as the seal of the legal person, power of attorney for receiving of wealth, personal passport.
The service is provided to individuals and legal entities.
Cost of service “SIM-card replacement” is 2500 rubles.

At replacement of a SIM-card in case of a loss telephone customer number, and also the tariff plan and a set of the services connected on a customer number remains the former.

“Setting the lock in case of SIM-card loss”

To install the lock it is necessary:
1. To call the Customer Care by number 909 - a free on-net number
+375 25 909 09 09 - for life:) subscribers at the rate of calls within life:) network, or
+375 17 295 99 99 - for life:) subscribers at the rate of call to other networks.
2. To provide a phone number or SIM-card number, which must be blocked.
3. To provide data to identify the subscriber:
∙ for individual subscribers - password or name and passport details of the owner of the contract;
∙ for corporate subscribers - the name of legal entity, legal address and name of the head. Also, it is desirable to provide bank account of a legal entity, name and bank code, Tax Payer Registered Number.
The service is provided to individuals and legal entities.
The service “Setting the lock in case of SIM-card loss” is provided free of charge.

“Removing the SIM-card lock”

To unlock the SIM-card it is necessary:
1. To contact life:) service center.
2. To provide a phone number or SIM-card number that you want to restore.
3. To provide documents to identify the subscriber:
∙ for individual subscribers - passport holder of the contract;
∙ for corporate subscribers - a statement on the service resumption of previously blocked SIM-card, certified by the signature of the director and chief accountant, and the seal of the legal person.
The service is provided to individuals and legal entities.
Cost of service “Removing the SIM-card lock” is 2200 rubles.
